PURPOSE
This document contains the approved working privacy wording for the public MJV Quote Form. It is not marketing consent and it must not be connected to direct public calendar booking.
A. SHORT PRIVACY NOTICE — PLACE IMMEDIATELY BEFORE THE SUBMIT BUTTON
Privacy Notice: By submitting this Quote Form, you authorize MJV Masonry & Contracting to collect and use the information you provide — including your name, contact information, project/service address, project description, preferred timing, and any photos or videos you choose to submit — to review your request, understand the project, contact you, prepare or discuss a quote, determine whether a site visit or consultation should be offered, provide related customer service, and maintain appropriate business records.
MJV Masonry & Contracting does not sell your personal information. Information may be shared only when reasonably necessary with authorized team members, subcontractors or trades, suppliers, and service providers that support quoting, project administration, communications, storage, hosting, forms, security, accounting, insurance, legal compliance or related business operations.
Photos or videos submitted with the Quote Form are for project assessment and related business purposes. Submitting a photo does not automatically give MJV permission to publish it for website, portfolio, social-media or advertising use.
Submitting this form does not book an appointment, create a contract, guarantee service, or create a final quote. Marty Vandenberg reviews Quote Form requests first. If a consultation or site visit is approved, MJV may privately provide an appointment or booking link afterward.

B. REQUIRED ACKNOWLEDGEMENT CHECKBOX
Recommended label:
☐ I confirm that the information I am submitting is accurate to the best of my knowledge, that I have permission to provide any property photos or project information included with this request, and that I have read the MJV Quote Form Privacy Notice and understand how MJV Masonry & Contracting will use the information to review and respond to my request.
The acknowledgement checkbox should be required before submission. It must NOT contain promotional marketing consent.
C. SEPARATE OPTIONAL MARKETING CONSENT
Do not add promotional email/text consent to the required Quote Form acknowledgement. If MJV later activates a promotional marketing program, use a separate optional, unchecked consent control that follows the approved CASL process. The current MJV Marketing Consent / CASL template remains inactive.
D. WEBSITE LINKING
The words “Privacy Policy” in the short notice should link to the public MJV Privacy Policy page. The public website Quote buttons should link to the Quote Form only. There must be no public calendar or booking link.
E. INTERNAL IMPLEMENTATION CHECK
Before launch, verify:
• the visible Quote Form fields match this notice;
• the privacy notice appears before submission;
• the acknowledgement is required;
• photo uploads, if enabled, match the notice;
• form responses go only to approved MJV business systems/accounts;
• the public Privacy Policy link works;
• no marketing checkbox is pre-selected;
• no public calendar-booking link or calendar connection exists.
